Booking an Appointment

If you are ready to begin with BLTG please contact admin@bltgpllc.com and we can schedule your initial appointment.  You can also select "Book an Appointment" on the home page and choose a potential appointment time. BLTG will confirm the time with you prior to the session.

 

Upon scheduling your session, you will receive a link to a client portal where you will enter your information. Forms include demographic information, insurance, credit card information, and others. Completing the requested information prior to the session is essential and will help us understand how to best help you or your child.  If you are unable to access the information for any reason, please let us know.  Paperwork will be available prior to your session.

Insurance and Fees

Insurance

 Brighter Life Therapy Group, PLLC and our providers are in network with Aetna, Blue Cross Blue Shield of Illinois PPO Blue Choice Options, and United Healthcare.

Out-of-Network Benefits

If we are out-of-network for your insurance we can provide you with a monthly superbill/receipt upon request.  Contact your insurance company if you have any questions regarding your insurance company's out-of-network benefits. Any reimbursement from your out-of-network insurance company will be sent to you. 

 

Payment for all out-of-network clients is due at the time of service.  

 

Payment

We accept cash, check and all major credit cards as forms of payment. 

 

Cancellation Policy

If you are unable to attend a session, please contact your clinician as soon as possible.  You may be charged a late cancellation fee for sessions cancelled less than 24 hours from scheduled counseling session.
